Developing a public health approach for implementation of the HL7 Clinical Document Architecture (CDA) Public Health Case Reporting (PHCR)

Statement of Work

The purpose of this project is to eliminate existing gaps in the electronic reporting of infectious diseases from clinical providers and local health agencies to state departments of health. CDC will address emerging needs of the ONC Standards and Interoperability (S&I) Framework to harmonize the Electronic Health Records (EHR) Meaningful Use (MU) requirements for reporting clinical summaries. The major steps of this project are:

  • Assess existing CSTE, PHIN and CDC documentation on state reportable and national notification for communicable diseases.
  • Assess different approaches to implement CDA reporting that might be from other perspectives, such as clinical care, cancer reporting and the National Healthcare Safety Network (NHSN).
  • Draft a public health approach to develop CDA implementation guides from healthcare (e.g., providers and local health departments) to state health departments by identifying the business case and leveraging the work from the ONC S&I Framework workgroups in the prioritization of use cases. This document might also include the approach to develop a national notification CDA mechanism from the state to federal level.
  • Test this approach through pilot projects with the identified CDA use cases with at least two states and two vendors/HIEs.
  • Work with internal CDC and external partners to develop the technical documentation required for these pilot projects.
  • Gather the data from the pilot projects and finalize the approach document, based on feedback from the use cases with states and vendors/HIEs, to provide recommendations on how CDA might be used for public health case reporting for both the healthcare to state level as well as the state to federal level.
